Server problem when changing map

I would like to know how to fix a server crash when it changes map... it just shows connection lost to server when it changes map, but the dedicated server still is working but not showing server in server list till some time... so if any1 knows the sollution then please reply

first of all: when the server is still working then "crash" is the wrong term. a crashed server does not run anymore 
does this happen on every mapchange to every map? or do you load a special map? maybe one which is very huge or uses many/large graphics/sounds and lua scripts?
what server are you using? cpu/ram/os?
